Patient safety investigations that actually improve care

Investigating incidents shouldn’t feel like an extra job on top of an already impossible one. Yet you’re expected to support families, guide staff, develop actions and produce reports, all while keeping services running.

Much of the training you’re given is abstract and disconnected from how healthcare really works. If you’re leading learning responses in an organisation delivering NHS-funded care, this training is mandatory. It should also be worth your time.

This course is built around a different premise: that good governance leads to insight, improvement and conversations that repair trust rather than assign blame.

Designed by NHS practitioners and Chartered Human Factors specialists actively working with PSIRF, it reflects the real pressures, trade-offs and constraints of healthcare systems, not an idealised version of them.

You’ll build the confidence, judgement and practical skill to lead patient safety investigations with compassion and impact, at a pace that fits around your work.

INSERT

No previous training is required.

If you’ve already completed Patient Safety Syllabus Level 1, this course builds directly on that foundation and focuses on practical PSIRF investigation and learning response skills.

If you haven’t completed Level 1, the course still stands alone. It provides the core concepts, context, and tools you need to lead and contribute to learning responses with confidence.

If you decide the course isn’t for you after completing the first module, you can request a full refund.

This gives you a genuine chance to assess the content, teaching style, and relevance to your role before committing further.

To keep this fair and workable, refunds are available provided:

  • the request is made after Module 1 and before starting Module 2
  • the request is submitted within 14 days of purchase

No justification is required. If it’s not useful, you shouldn’t be stuck with it.

You’re not left to figure things out on your own.

We’ve built long-standing relationships with healthcare professionals through our face-to-face training, and we bring that same approach here.
